Group Facilitator
We are currently seeking a Group Facilitator/ Social Service Tech 2 to join our team in making a difference in the lives of our patients every day. Summary of Duties
Group Facilitator's/Social Service Tech 2 joining our team must have strong communication skills to work with patients and staff as part of our multi-disciplinary team. Good organizational skills are required. This position is responsible, positive role modeling, documentation of flows, crisis intervention and patient safety.
Responsibilities
Under general supervision, provides a broad range of paraprofessional social services to patients/clients/consumers and their families.
The individual in this role will provide direct care. Responsibilities will include but not be limited to the following:
- Conducts patient/client/consumer intakes, participates in treatment team planning, and performs crisis intervention
- Assists in dealing with personal and social problems.
- May provide supportive counseling to consumers and families and/or serve as a liaison for social services and may perform case management duties
- Assists clients in locating and utilizing community resources including legal, medical, financial assistance, and other referral services.
- Completes patient/client/consumer intakes based on interviews with patients/clients/consumers, their families, significant others and appropriate community agencies
- Counsels clients living in group homes and halfway houses; assists in pre-release and release planning.
- Implements and organizes the delivery of specific social services within the community.
- Implements life skills workshops and programs in behavior management, youth services, community and social services.
- Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress.
- Monitors patient's/client's/consumer's progress towards treatment goals.
- Maintains program statistics for purposes of evaluation and research.
- Prepares intake reports and case history records and assist with discharge planning
- Provides short-term case management and referral services to clients with emergency situations
- Actively participates in assigned and appropriate meetings as required to discuss needs, goals, objectives, and outcomes of services, quality assurance, etc.
- Provides education to client and family.
- Flexible and adaptable. Be willing to work at multiple site

Minimum Qualifications:
- High school diploma or GED
- Associates degree in a social services related field from an accredited college or university
- Two (2) years of experience in a Social Services Tech 1 position
- Two (2) years’ experience in Behavioral/Mental Health Field
- At least 21 years of age
- Possess a valid driver’s license
- Outstanding interpersonal skills and the ability to work with patients of all ages
- Submit to and pass a pre-employment drug screen and background checks
Preferred Qualifications:
- Bachelor's degree in a social services related field from an accredited college or university
- Three (3) years work experience in Behavioral Health / Mental Health field.
- Relevant work experience in Behavioral health patient care is highly preferred.
