We are currently seeking a Group Facilitator/ Social Service Tech 2 to join our team in making a difference in the lives of our patients every day.  We are located in the heart of Middle Georgia State where we enjoy abundant recreational opportunities, and a family friendly environment with good schools and opportunities for the entire family to be involved.

Summary of Duties

Group Facilitator's/Social Service Tech 2 joining our team must have strong communication skills to work with patients and staff as part of our multi-disciplinary team. Good organizational skills are required. This position is responsible, positive role modeling, documentation of flows, crisis intervention and patient safety.

Responsibilities

Under general supervision, provides a broad range of paraprofessional social services to patients/clients/consumers and their families.
The individual in this role will provide direct care. Responsibilities will include but not be limited to the following:

  • Conducts patient/client/consumer intakes, participates in treatment team planning, and performs crisis intervention
  • Assists in dealing with personal and social problems.
  • May provide supportive counseling to consumers and families and/or serve as a liaison for social services and may perform case management duties
  • Assists clients in locating and utilizing community resources including legal, medical, financial assistance, and other referral services.
  • Completes patient/client/consumer intakes based on interviews with patients/clients/consumers, their families, significant others and appropriate community agencies
  • Counsels clients living in group homes and halfway houses; assists in pre-release and release planning.
  • Implements and organizes the delivery of specific social services within the community.
  • Implements life skills workshops and programs in behavior management, youth services, community and social services.
  • Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress.
  • Monitors patient's/client's/consumer's progress towards treatment goals.
  • Maintains program statistics for purposes of evaluation and research.
  • Prepares intake reports and case history records and assist with discharge planning
  • Provides short-term case management and referral services to clients with emergency situations
  • Actively participates in assigned and appropriate meetings as required to discuss needs, goals, objectives, and outcomes of services, quality assurance, etc.
  • Provides education to client and family.
  • Flexible and adaptable. Be willing to work at multiple site

*All applicants are subject to provide a current seven (7) year Motor Vehicle Report and Georgia background check.*

Minimum Qualifications:

  • High school diploma or GED
  • Associates degree in a social services related field from an accredited college or university
  • Two (2) years of experience in a Social Services Tech 1 position
  • Two (2) years’ experience in Behavioral/Mental Health Field
  • At least 21 years of age
  • Possess a valid driver’s license
  • Outstanding interpersonal skills and the ability to work with patients of all ages
  • Submit to and pass a pre-employment drug screen and background checks

Preferred Qualifications:

  • Bachelor's degree in a social services related field from an accredited college or university
  • Three (3) years work experience in Behavioral Health / Mental Health field.
  • Relevant work experience in Behavioral health patient care is highly preferred.

Bachelor's degree in a social services related field from an accredited college or university OR High school diploma or GED AND Two years of experience in a social services related position OR One year of experience at the lower level Social Svcs Tech 1 (SST010) or position equivalent.
