Peer & Psychosocial Rehabilitation Team Lead
Pathways Center’s mental health Peer Support Program is designed to help adult persons with psychiatric disabilities/disorders or other behavioral health needs to optimize their personal, social, and vocational competency in order to live successfully in the community. Participation is determined by the needs of the person served. The programs are designed for individuals that are age eighteen years and older and who have severe and persistent mental illness and/or co-occurring disabilities. The program provides structured activities within a MH Peer Support program that promotes socialization, recovery, wellness, self-advocacy, development of natural supports, and maintenance of community living skills.
The responsibilities for the Peer/PSR Team Lead include but not are not limited to the following:
□ Organizes and directs program operations to meet client needs, to effectively operate within state and federal regulations, certification guidelines, and to ensure compliance
□ Applies a variety of appropriate psychotherapeutic and counseling techniques to improve behavioral functioning of clients
□ Participates in team staffing and treatment plan development
□ Documentation within required time frames to reflect interventions and the clients progress towards his/her goals, and overseeing all team members in the Peer/PSR-g programs are meeting documentation standards and guidelines.
□ Transports clients to and from the program as needed
□ Development and implementation of structured groups and community outings
□ Effectively designs and coordinates protocol and procedures for identifying client service needs
□ Development and execution of daily lesson plans to teach socialization, problem solving and coping skills; Illness and medication self-management; prevocational skills and self-advocacy; and recreational activities/leisure skills that improve self-esteem and recovery
□ Thoroughly reviews program guidelines, criteria, and scope of services with clients and their families to ensure their understanding of services.
□ Completes service authorizations on all clients to ensure services are approved by the appropriate payer.
□ Leads recreational and leisure activities that improve self-esteem and recovery
□ Attends clinical supervision and professional development opportunities as applicable
□ May perform some case management duties to facilitate and ensure communication, linkage, and continuity of care between individuals, physicians, other organizational components, community resources and service providers.
□ Write timely progress notes that relate directly to the treatment outcomes.
□ Provide individual, family or group therapeutic counseling and/or educational and skill development sessions as indicated in the service plan.
□ Provides training to direct care team members in implementing behavior modification techniques and treatment/service plans
Bachelor's degree in a related field from an accredited college or university.
Note: Some positions require eligibility for licensure.
