+This is a 2nd Shift position, typically 3pm to 11pm.
Pathways Center provides community-based services in Greenville, GA (Meriwether County) that support individuals with diagnosed with Intellectual & Developmental Disabilities (IDD). The care is provided on a time-limited basis. The outcome of these services, help to enhance the family and/or caregiver’s ability to meet the needs of the individual and minimize the need for the individual to leave his/her home in order to resolve the presenting crisis. Intensive In-Home Support services include, but are not limited to, the following: Implementation of behavioral intervention strategies provided under the recommendations of the intensive in-home provider, safety plans, or behavioral support plans already established for the individual; provision of one-to-one support, as necessary, to address the crisis; modeling of interventions with family and/or provider staff; assistance with simple environmental adaptations as necessary to maintain safety; and when necessary accompanying the individual to appointments related to the crisis response.
Pathways Center operates a home based in Greenville, GA that serves up to four (4) individuals who are experiencing an emotional/behavioral change and/or distress that leads to a disruption of essential functions. These Out-of-Home services help to stabilize the individuals so they can return to their own home environment.
Responsibilities for the Licensed Practical Nurse (LPN) will include but not be limited to the following:
- Work with the treatment team to provide direct care to individuals residing in Crisis Home based upon the individual treatment plan
- Measures and records clients' vital signs, such as height, weight, temperature, blood pressure, pulse and respiration
- Care includes providing medical treatments, administering medication and giving injections.
- Helps individuals with bathing, dressing, and personal hygiene, moving in bed, and standing and walking
- Care may also include meal preparation, and light custodial duties
- Observes individuals in Crisis Home
- Documents and reports any changes in client’s conditions, such as adverse reactions to medication or treatment, changes in behavior, etc. Notifies appropriate treatment team members and discusses any necessary action…
- Participates in developing and reviewing appropriate treatment plans
- Transports or makes arrangements for transportation of clients to appointments in a timely manner
- Provides appropriate information regarding psychological stresses and level of global functioning identified in the client's record
- Promptly notifies appropriate professional of any unusual client incident or situation
- Recognizes the need for client crisis intervention and reacts appropriately to routine situations and/or promptly notifies person on call or immediate supervisor
- Must possess positive interpersonal skills with internal and external stakeholders
Minimum Qualifications:
Vocational/Technical degree from an accredited college AND current Georgia license to practice as a Licensed Practical Nurse AND One year of nursing experience.Equal Employment Opportunity Employer
