Job Description:
Pathways Center’s Care Campus has two Crisis Stabilization Units; one for adults ages 18 and older, and one for children & adolescents ages 5-17. Pathways Center's Crisis Stabilization Units (CSU's) are short term (2-7 days), medically monitored crisis residential programs, which offer services for a limited duration to stabilize acute psychiatric or behavioral symptoms, evaluate treatment needs, and develop plans to meet the needs of persons served. The programs operate 24 hours per day 7 days per week. The CSU's are designed to serve as a first line of community based alternative care to hospitalization, offering psychiatric stabilization and detoxification services on a short term basis.
In addition to the crisis stabilization programs, Pathways Center Care Campus has a Crisis Service Center (CSC). The CSC provides on-site and telephonic crisis intervention and 23-hour observation services 24 hours a day, 7 days per week. The CSC operates on a walk-in basis and provides psychiatric/substance abuse related crisis evaluation and brief intervention services to support an individual who is experiencing an abrupt and substantial change in behavior noted by severe impairment of functioning typically associated with a precipitating situation or a marked increase in personal distress. Interventions are provided by licensed behavioral health professionals designed to prevent out of community treatment or hospitalization.
The Licensed Practical Nurse (LPN) assists the Registered Nurse in providing oversight for medications and coordinating the medical services. Duties include but are not limited to the following:
- Provide general nursing care to clients as needed
- Assists in managing the medication administration process, medication orders, and coordinates with the on-site pharmacist regarding inventory, prescriptions, etc.…
- Accurately and efficiently utilizes the company’s electronic medication administration record (MAR) system
- Provides nursing services in conjunction with the physician, physician assistant, and/or nurse practitioner in core services and acts as a liaison to communicate pertinent information to the medical team including participation in daily team staffing with other clinical team members
- Measures and records client’s vital signs, such as height, weight, temperature, blood pressure, pulse and respiration
- Observes clients, charting and reporting changes in client’s conditions, such as adverse reactions to medication or treatment, and taking any necessary action
- Completes clear and concise required documentation for the patient’s/client's chart utilizing the organization's Electronic Health Record (EHR) system and in accordance with established policies and procedures and within specified timeframes
- During walk-in triage, in the absence of or in conjunction with the RN, provides a medical assessment including history and physical; medical history symptoms review; cursory head to toe exam; drug test; pregnancy test (if applicable); vitals; breathalyzer; heart/lung sounds; and blood pressure
- Works as part of a health care team to assess client needs, plan and modify care, and implement interventions.
Minimum Qualifications:
Vocational/Technical degree from an accredited college AND current Georgia license to practice as a Licensed Practical Nurse AND One year of nursing experience.Equal Employment Opportunity Employer
