Licensed Practical Nurse (LPN) – Outpatient Behavioral Health
Carrollton Outpatient Clinic | Carrollton, Georgia
Full-Time | 4-10 hour days | No Nights, Weekends, or Major Holidays

Pathways Center is seeking a compassionate, organized, and team-oriented Licensed Practical Nurse (LPN) to join our Outpatient Behavioral Health team based in Carrollton, Georgia. This position provides nursing support to individuals receiving outpatient mental health and substance use treatment while working collaboratively with psychiatrists, nurse practitioners, physician assistants, therapists, case managers, and other members of the interdisciplinary treatment team.
Although based in our Carrollton Outpatient Clinic, this position provides nursing coverage at multiple Pathways Center outpatient clinic locations throughout the service area as needed.

What You'll Do
As an Outpatient Behavioral Health LPN, you will:
  • Provide nursing care and support for individuals receiving outpatient behavioral health services.
  • Complete nursing assessments, collect vital signs, and document findings in the electronic health record.
  • Assist psychiatric providers with patient visits and clinical procedures.
  • Administer prescribed medications, including long-acting injectable medications, in accordance with provider orders and nursing standards.
  • Monitor clients for medication effectiveness, side effects, and adverse reactions, communicating concerns promptly to the prescribing provider.
  • Provide medication education and reinforce treatment plans with clients and families.
  • Perform medication reconciliation and assist with prescription refills as directed.
  • Coordinate laboratory testing, drug screens, TB testing, and other clinical services.
  • Process prior authorizations and coordinate with pharmacies and patient assistance programs to ensure timely medication access.
  • Maintain examination rooms, medication supplies, vaccines (if applicable), and medical equipment.
  • Collaborate closely with psychiatrists, therapists, case managers, peer specialists, and other healthcare professionals to support integrated care.
  • Respond appropriately to urgent behavioral health concerns and follow agency protocols for crisis situations.
  • Travel to multiple outpatient clinic locations to provide nursing coverage and support.
Minimum Qualifications
  • Current Georgia Licensed Practical Nurse (LPN) license in good standing
  • Current CPR certification (or ability to obtain upon hire)
  • Valid Georgia driver's license with reliable transportation for travel between clinic locations
  • Strong communication, organizational, and customer service skills
  • Ability to work effectively within a multidisciplinary behavioral health team
Preferred Qualifications
  • Experience in behavioral health, psychiatry, substance use treatment, community mental health, or outpatient healthcare
  • Experience administering long-acting injectable medications
  • Experience with electronic health records
  • HCV and HIV Certified Tester (or willingness to obtain if required)

Work Location
Primary Location: Carrollton Outpatient Clinic – Carrollton, GA
This position is based in Carrollton and requires regular travel to other Pathways Center outpatient clinic locations within the service area to provide nursing coverage and support.

Minimum Qualifications:

High School diploma/GED and completion of a LPN Program through an approved Georgia Nursing Board; or Vocational/Technical degree from an accredited college and current Georgia license to practice as a Licensed Practical Nurse and one (1) year of nursing experience; or one (1) year experience required at the lower-level Licensed Practical Nurse 2 (HCT031) or position equivalent.
