Job Title: Planning List Administrator- Region 2 Field Office- Augusta

Job Summary:
The candidate chosen for this position will perform the duties listed below in multiple counties in North Georgia. The position is located at the Region 2 Field Office in Augusta, Georgia.
Must be a highly organized individual, as this position will require a significant amount of tracking and follow-up for many different job functions
Requires completion of a significant number of Needs Assessments for new individuals as well as annual renewals for individuals on the Planning List
Requires telephone support to individuals and families on the Planning List, and bringing resolution to phone calls and messages
Requires reporting Planning List changes and concerns to Planning List Supervisor
Make and resolve referrals to outside agencies for non-waiver services
Other supports for Planning List as needed.
Qualifications:
Bachelor’s degree, Human Services field preferred. Experience working with individuals with intellectual/developmental disabilities is strongly preferred. Must have at least an intermediate level of computer knowledge, Office knowledge preferred. Must report to Region Two Field Office in Augusta, Georgia.
Preferred Qualifications:
Experience with or knowledge of individuals with intellectual/development disabilities, Medicaid waivers, and DBHDD I/DD services; experience working successfully without daily contact with a supervisor.
